Leaving UAMS

Be sure to notify both your supervisor and Human Resources at least two weeks in advance of your intent to leave UAMS and resign your current position.

Pick one of the following three ways to do this:

  1. PREFERRED:  Complete an online form in Employee Self Service. The form is located in the Career & Job section.  Click “I Am Resigning from UAMS” and provide your last day of work and reason for leaving.  Then “Save Changes” to officially notify both your supervisor and Human Resources.  If the date changes before the final employment date, return to this page and select “Delete This Notice.”
  2. OR hand deliver a written statement to Human Resources, located on the 4th floor, 4C, of the Central Building. Human Resources will process your request in Employee Self Service for you.
  3. OR email Human Resources will process your request in Employee Self Service for you.

Your supervisor will notify you of the additional separation clearance steps.

What Happens to My Benefits When I Leave UAMS?

Continuation of Medical, Dental or Vision insurance for up to 18 months for all exiting employees (and those who change to non-benefits status)

Update your Address
Update your address with UAMS and benefit vendors.  Final terminal vacation checks and W2s will be mailed to the address on file with UAMS.  If that address has changed, please log into Employee Self Service to update.

32 Day Form-Retirement Distribution
Must be signed by an employee who wishes to access their TIAA or Fidelity retirement plan benefits within the first 32 calendar days after leaving UAMS employment.  Before “cashing out” your retirement plan, we recommend you first contact your fund sponsor, TIAA or Fidelity.  If you decide to “cash out” your TIAA or Fidelity retirement account you much complete a release form and contact TIAA or Fidelity.  Please note, it is advised that you wait at least 45 days after your final check is received to ensure all retirement funds have been deposited into your account.  Otherwise, the “cash out” process may have to be completed again.

Vacation and Holiday Payout
Once the clearance process is complete, Payroll will process the check on the next payroll cycle. The check will be mailed to your home. This payout could take between 3 – 4 weeks after the clearance process is complete.  If your vacation/annual leave is paid out, the employee will not be able to return to University employment until the number of days for which he/she received additional compensation has expired. Employees rehired or transferring to another State agency or institution may transfer all accrued annual leave. Please refer to Administrative Guide 4.6.04.
