The UAMS HR Department has several forms available for you to access online. Click on the links to the left to find the type of form you need. All benefit forms can be completed and faxed to 501-686-5386 or emailed to ASKHR@UAMS.EDU. Always remember to keep your fax confirmation sheet.
Employees needing assistance may call us at (501) 686-5650 during our office hours – 8:00 a.m. through 4:30 p.m. Monday through Friday, by emailing us at ASKHR@UAMS.EDU, or by visiting our office between 7:30 am and 4:30 pm on the 4th floor of Central Hospital 4C.
